How It Works

Tryptyr helps your eyes make more of their own natural tears instead of just adding artificial tears.
  • It activates cold-sensing TRPM8 receptors on nerves on the surface of the eye.
  • These nerves send signals that increase your baseline (everyday) tear production.
  • More natural tears help keep the eye surface moist and comfortable, reducing dryness and irritation.
.

Treatment and Efficacy

Approved indications: Tryptyr (acoltremon ophthalmic solution 0.003%) is indicated for the treatment of the signs and symptoms of dry eye disease in adults.

Off-label uses: As a newly approved medicine, there are no well-established off-label uses; any use outside dry eye disease should be considered experimental and guided by an eye specialist.

Efficacy expectations:

  • In clinical trials, many patients showed increased natural tear production as early as day 1, with more noticeable improvements by about 2 weeks.
  • A substantially higher proportion of patients on Tryptyr achieved meaningful increases in tear production compared with those using placebo eye drops, and benefits were maintained over about 3 months in studies.
  • Compared with some older prescription dry eye drops that may take weeks or longer to work, Tryptyr tends to have a faster onset of effect, though not all patients will respond and some may still need other or additional therapies.

Dosage and Administration

Typical dosing:

  • Adults, including older adults: instill one drop of Tryptyr in each affected eye twice daily, about 12 hours apart (for example, morning and evening).
  • The same dose is used regardless of dry eye severity, unless your eye care professional advises otherwise.
  • Safety and effectiveness have not been established in children, so routine pediatric use is not recommended.

How to use the eye drops:

  • Wash your hands thoroughly before handling the vial.
  • Open a single-dose vial and, while tilting your head back, gently pull down the lower eyelid to form a small pocket.
  • Hold the vial above the eye without touching the eye or eyelids, and squeeze out one drop into the pocket; repeat for the other eye if prescribed.
  • Close your eyes gently for a short time after the drop to help it spread; do not blink hard or rub your eyes.
  • Use each single-dose vial only once for both eyes, then discard it immediately.
  • If you use other prescription eye drops, space them at least 5 minutes apart, using Tryptyr in whichever order your eye care professional recommends.
  • Remove contact lenses before use and wait at least 15 minutes after dosing before reinserting them.

Special dosing instructions:

  • Use the drops regularly at the same times each day for best effect, even if your eyes start to feel better.
  • Do not use more than your prescribed number of daily doses unless your prescriber specifically tells you to.
  • Tryptyr is for use in the eyes only; do not swallow or inject it.

Missed dose: If you miss a dose, apply it as soon as you remember unless it is almost time for your next scheduled dose; if it is close to the next dose, skip the missed one and return to your regular schedule without doubling doses.

Overdose: Accidentally applying extra drops to the eye is unlikely to cause serious problems beyond more local irritation; if a large amount is used or the solution is swallowed and you experience concerning symptoms, contact your healthcare provider or a poison control center for advice.

Safety and Side Effects

Common side effects:

  • The most common side effect is eye pain or burning/stinging at the time of instillation; this is very common (around half of patients in studies) but is usually mild to moderate and brief.
  • Other commonly reported effects include temporary eye irritation, redness, or blurred vision right after putting in the drops.
  • Less than 1% of patients in trials stopped the medication because of burning or stinging.

Serious or rare adverse effects:

  • Serious eye problems were uncommon in studies, but you should seek urgent medical care for severe or worsening eye pain, sudden major changes in vision, marked redness or swelling, or thick discharge, as these may signal infection or other eye disease.
  • Allergic reactions to the drops are possible but appear rare; symptoms could include rash around the eyes, swelling of the eyelids, or intense itching.

Warnings and precautions:

  • To avoid eye injury or contamination, do not touch the vial tip to your eye, fingers, or any surface.
  • Do not use Tryptyr while wearing contact lenses; remove lenses before dosing and wait at least 15 minutes after putting in the drops before reinserting them.
  • Pregnancy: human data are limited, but systemic absorption from the eye is very low; discuss risks and benefits with your healthcare provider if you are pregnant or planning pregnancy.
  • Breastfeeding: it is not known if acoltremon passes into breast milk, but systemic exposure is low; your provider will help weigh the benefits of breastfeeding and your need for treatment.
  • Pediatrics: safety and effectiveness have not been established in children, so it is not routinely used in pediatric patients.
  • Kidney or liver disease: because blood levels after eye dosing are negligible, dose adjustments are generally not expected for these conditions.

Overall safety profile: Tryptyr has very low systemic absorption, so side effects are primarily local to the eye, and systemic adverse effects and drug interactions are unlikely compared with some systemic or immune-modulating dry eye treatments.

Reporting side effects and safety updates: Side effects can be reported to the manufacturer using the phone number on the package insert or to the U.S. FDA MedWatch program (online or by phone); updated safety information is posted on official labeling and regulatory websites as it becomes available.

Interactions and Precautions

Drug and product interactions:

  • Because systemic absorption of Tryptyr from the eye is very low, clinically important interactions with most oral prescription drugs, over-the-counter medicines, supplements, foods, or alcohol are unlikely.
  • When using other prescription or over-the-counter eye drops, separate them from Tryptyr by at least 5 minutes to prevent one drop from washing out the other.
  • Always remove contact lenses before instilling Tryptyr and wait at least 15 minutes before reinserting them, as advised in the product instructions.
  • No specific interactions with imaging contrast agents or diagnostic eye procedures have been identified, but inform your eye care professional that you are using Tryptyr before exams or procedures.

Precautions and conditions requiring caution:

  • Tell your eye care professional if you have active eye infections, recent eye surgery, eye trauma, or severe glaucoma so they can decide if Tryptyr is appropriate and how it should be used.
  • Inform your healthcare provider about pregnancy, plans for pregnancy, or breastfeeding, since human data are limited even though systemic exposure is low.
  • Because safety and effectiveness are not established in children, use in pediatric patients should only occur under specialist guidance.
  • People with known allergies to any component of the formulation should not use Tryptyr.

Monitoring:

  • Routine blood tests or heart monitoring are not required with Tryptyr because systemic exposure is minimal.
  • Regular follow-up eye examinations are recommended so your eye care professional can assess symptom relief, tear production, and the eye surface, and adjust treatment if needed.

Common Questions and Answers

Q: How quickly will Tryptyr start to work for my dry eye symptoms?
A: In clinical studies, many patients had increased natural tear production within the first day, and symptom improvement often became noticeable over days to a few weeks, though individual responses vary and some people may not experience strong relief.

Q: How long do I need to keep using Tryptyr?
A: Dry eye disease is usually chronic, so Tryptyr is often used long term as directed by your eye care professional, who will periodically reassess whether you should continue, adjust, or stop treatment.

Q: Can I use Tryptyr with my other prescription eye drops?
A: Yes, Tryptyr can be used with other ophthalmic medicines, but you should wait at least 5 minutes between different eye drops and follow your prescriber’s instructions on the order of use.

Q: Is Tryptyr safe to use with contact lenses?
A: You should remove contact lenses before putting in Tryptyr, then wait at least 15 minutes after dosing before reinserting them, to reduce the risk of irritation and contamination.

Q: What if I miss a dose of Tryptyr?
A: If you forget a dose, use it as soon as you remember unless it is almost time for the next one; if it is close to the next dose, skip the missed dose and return to your usual schedule without using extra drops.

Q: Can I use Tryptyr if I am pregnant or breastfeeding?
A: Human data are limited, but very little of the drug enters the bloodstream after eye use, so your healthcare provider will weigh the potential benefits of treating your dry eye against any unknown risks during pregnancy or breastfeeding.
